• Comprehensive, personalized care – longer appointments and detailed wellness planning

• Enhanced access to providers – same day or next day appointments, minimal wait times, and priority scheduling

• 24/7 after-hours communication – direct access to Dr. Chafin or Stacey Brausch, AGNP-BC via cell phone, text, or email during non-office hours for urgent needs

• Proactive preventive care – yearly wellness examinations, lab screenings, health risk assessments, and individualized wellness plans

• Coordinated care with specialists – the practice assists with referrals and communication with other medical professionals

• Prescription management and support – including timely refills and medication guidance

• Yearly flu shots – offered to all members between September and February

• Comprehensive annual physical and wellness exam

• Lab screening including CBC, TSH, CMP, PSA, A1C, advanced cholesterol testing, urinalysis

• Women’s yearly Pap smear

• Health risk assessment and personalized wellness plan

• Unlimited scheduled office visits (with prior scheduling)

• Prescription refills (allow 24–48 hours for processing)

 

Non-Medical, Personalized Services:

• Direct access to providers with minimal wait times

• Extended office visits when needed

• HIPAA-secured email and messaging for non-urgent communication
